I'd bet anything that it was mechanical failure.

Looks like Marlin Club — Shelter Island Drive.
Summer time... Overjoyed boat owners of $10 million + $$ yachts saunter down Mexican coast on weekends. . .
BINGO! They latch a "Keeper Marlin." Great for bragging rights! Then "Hi-Ball" it back to Marlin club by 1500 hrs. on Sunday.... Not a lot to do on a yacht with $25,000 auto navigation "gear." Next best thing?¿ ?¿? . . . . BREAKOUT THE BELLY WASH! < < Do that for 6 hrs. . . I've seen that same act many times - 1957 era - when attached to Submarine Warfare College - Sonar School - short distance from Marlin Club. . . . Not really a big deal. Patch up bow... $ $ $ $ FOR ANOTHER RUN! — 'AWAY WE GO!'

The report I saw said that the throttle has stuck or jammed and that it was at full throttle which I doubt. No captain would be approaching a slip at full throttle. The engine would have been set to give him only steerage speed because he'd know he had only the length of the slip to stop the forward motion.

Slamming the transmission into reverse at "full throttle" after the throttle problem occurred might have slowed the boat somewhat or more likely just destroyed the transmission having no effect on the speed or crash. (Try it on your automatic transmission automobile and see how well it works.)
