In the Library module right click on the folder and select Synchronize. This will update the LR catalog to account for pictures deleted outside of LR.

If you create/change/delete a file from My Pictures you MUST do it in LR4 and not from Windows Explorer. If you do this from LR4, it will keep your catalog straight all the time.

No, it's not automatic. While in the Library module, select My Pictures on the left side, choose Library from the top left tool bar, and select Synchronize Folder. That's a manual update.

If you have subfolders in My Pictures you can choose a subfolder in it if that's all you changed, then all of My Pictures does NOT update which could take a long time if you have a lot of files in My Pictures.
