It also appears to me that there is a focus problem. Check out the lens AF or MF setting. If it is on manual and the camera is on auto, that would also explain some of the focus problem, also check the dioptric adjustment knob. I had problems with that until i found out it WAS that.

The focus "problem" is more to do with 1/3d of a second, which is IMPOSSIBLE, also I guess he has ALL focus points enabled as you can clearly see it is focussing on the background hence sharp large red letters and numbers

Hi Dickie. These are actually pretty good for the settings you are using. I could never hold the steady at those shutter speeds. I think you have to use a tripod if you are that far away from them to have to use the 200mm. Or get closer so you can use the flash. But I don't know the limits on your flash either. But you have to get the shutter speed up, I would push the ISSO as far as you can. Someone might know what that would be here. If you go to high with it the same thing will happen. The pictures will be noisy.
I would play in the house with settings in a room that the lighting was close to the schools. Start at 400 and go up. If I had to guess, you might be able to get good pictures at ISO 800 if you need to go that high.

Welcome. You have gotten some very good advice so far. I would suggest that if you can, get into the venue prior to the concert with your daughter, and take some test shots. Check your focus, ISO, and DOF (depth of field) settings. You may be shooting too "wide open" (large aperture) and that might be contributing to the soft focus. Try shooting as high an ISO and shutter speed as you can get away with.

I think JR1 is right, 1/3 second is far too slow even using a tripod. Often stage lights are quite bright and with a higher iso might be enough. If the group has a dress rehearsal consider taking your photos there. You won't have to contend with an audience. Unless you have a very strong flash unit, Canon 580 or similar, you may find a single flash not much help. Setting the white balance may help, go to a rehearsal and check the lighting.

Are you using center focus point or using the focus array?

The array, especially in lower light will often grab a nearer focal point than your subject. Try center point only...

Up your shutter speed to 200 and then make it slower until pics get fuzzy then click it back up one... Remember when kids are moving up there "IS" does minimal to help stop action, it just corrects your movements. I think that you will need at least 1/60(somebody confirm that for me) to catch some sharp stills with small movements of subject and still most of those will be a miss...

bump your ISO to 3200 and come back down to lowest optimal setting. Proper white balance will minimize some of the noise, but unless you are sporting a 2.8 or wider, you will have to put up with some noise (if stage is brightly lit, noise will be minimized)

You can do all of this while they are adjusting their instruments before the concert master comes on...

also
Higher ISO allows your flash to be usable further out...
