Are these just tree holes?

I do know that number 4 is the work of Woodpeckers. Probably Pileated Woodpecker, or, as we call them
in Western NC "Woodhens". I've seen the same shape on trees they were working on.

This photo from the internet. Number 5 could be from the same culprit.

I've never seen a square tree hole before. From a Pileated or any other bird. It doesn't look natural to me. But maybe something came along and took advantage of it to hollow out the inside after the hard work was done!
