Number 13, the toilet in the elevator, comes to mind. I'm not sure what folks in a crowded elevator would think if someone just drop "trou..." and did their "business".

41. Lisbon, Portugal has an open-air escalator (why aren't there more of these???).

The elevated portions of the Long Island Rail Road has open-air escalators flanking the stairs. Sometimes multiple sets at a station.
Of course, the question arises: why would anyone in a store want to go outside into the weather to go from one floor to another? Or if these are municipal escalators, who's going to pay for the upkeep and maintenance of them?

Remember when someone let out a squeaker in an elevator and everybody wondered who did it? No more guessing, it's the man sitting in the corner.

If any of these features cost money, we will not see them in American. Businesses "can't afford" to spend money.

If you are ever in Hong Kong, check out the open air escalator from Central up to the Mid-Levels. It is more than 800 meters in length and the elevation change is more than 135 meters from bottom to top.
