I use a Sirui L-20S; same tilt portion as the L-10 but has a panning base. I like the panning base because I use it on a monopod with legs, and prefer to let the rotation be at the head versus the base.
I also use a Desmond DMH-1 ($37), which is just the tilt portion, and I've added a DAC-X1 arca clamp ($15) to the top. This can be screwed onto the monopod, or clamped into a panning clamp. You can get the DMH-2 which is similar but includes the clamp for $60.
I prefer the Sirui, it has a better clamp and smoother friction adjustment.
I've used a Manfrotto Pistol -Grip head for years. Also , now have a Gimble head for tripod use.
This is exactly what I was gonna tout. The Wimberly Monogimbal. I think it’s around $180 but it’s a great piece of equipment. Much better than a ball head for that big lens.
I’ve been pretty satisfied with the Desmond DMH-2, a RRS knockoff. $60 on Amazon with arca clamp. They claim a 22lb. capacity. The interesting thing about this is that the base is square and sized to fit a clamp as well, although i have not used it as such.
I did replace the clamp with an RRS lever clamp, which would hike the price, of course.
I bought the Wimberley MH 100 gimbal head and have found it to be the best monopod head I have tried and I have tried some of the cheaper heads and there is no comparison. Check out backcountry gallery he did a review on utube

Not a good idea. Especially for a long lens, NOT RECOMMENDED.

Exactly.
Yaw is just pivot,
Not much roll needed,
pitch would be the most adjusted,
which a tilt head would accomplish.
I also use the Desmond DHH-2 with a arca clamp with great results. About $60 bucks. I bought based on a thread about 1 1/2 years ago on UHH and very glad for whoever recommended it.
Steve Perry did a story about a head for a monopod. Sorry I don't know the particulars but it shouldn't be hard to track down.
