The cell phone case is for iPhone XS but the lenses can be used with other cell phones after obtaining the corresponding case. I found myself carrying and using a pocket camera instead. I am not sure about the case fitting iPhone 11 Pro Max but don't think so based on Moment's website:

I don't recall what was the original packaging but I commonly only keep the essentials to minimize clutter.
