When I import my photos from memory card, LR asks the destination but once I have given the proper destination folder it creates an entire subfolder system with year, then year and time. How do I just have it import the photos without the added folders?

You need to go through your import settings.
I have a year folder and all new downloads go into a sub folder with the date. If I do multiple imports at first they show as separate but shortly there after only appear in one folder of the date the shots were taken.
Oh, on one import if you have shots taken say two days in a row at first they appear together but if you check the file tree you will find two date folders (or more as the case may be).
And you can rename the folders, split them, merge them etc. But do it all in LR or LR will lose track of them.
What import settings do I need to apply? I don't find an option on folder structure or whether or not LR will create folders for me. But it does anyway. Note, I only want to create my own folders in LR, not what LR thinks my folder structure will be.

Give us a screenshot of your import settings

That's not a bug, it's a feature. It enables you to find an image based on date shot, which can come in handy. There's no duplication of files, just a different way for the LR database to display data.

In the Destination Section, uncheck "Into Subfolder"

I agree, I have about 6 TB of images and they would be nearly impossible to search if I didn't have them sorted by DATE and TIME shot. Plus, I can use key words to make searching easier too but the DATE and TIME are my fall backs. Otherwise my storage would be a real mess.

Simple answer: Before you click Import in the Lower Right Corner look at the Destination box and uncheck Into Subfolder. While your there click on the Organize box and change it from "By date" to "Into one folder".

Those are your preferences not your import settings. As someone has done a screenshot of their import settings, you should click on the Import button and then carefully examine all the different choices you have in the RIGHT hand panel. Go through them all and make sure you are telling LR what you want it to do in that panel.
