ok, I purchased a EF L 100-400 usm lens and already had a 1.4 extender for the L series lens, I also have 2-2x extenders for my other Canon lens. Long story. My Canon 5ti loses the ability to control the AF because of light and f-stop limitations. So, I mount my lens on a tripod and go to shoot moon shots the first is with just the 100-400 with the 1.4 ex the second is with the 100-400 with 1.4 and both 2x and I had to back off to 250mm to get the full moon into frame. I had the camera on 2 sec delay, and was focusing with the led screen because of reach to focus ring on lens. the last picture is a hand held shot from earlier in the day cropped.

f/14 1/100 800 ISO
f/14 1/100 800 ISO...

f/18 1/100 400 ISO

f/16 1/60 800 ISO

Normally two extenders is the absolute max that you can go. Even then the image quality may suffer enough that you do not want to go that route. Plus with the extender built into the lens it may make the image unacceptable because you can not change the positions of the extenders. I am not sure why the extended exposure came out that way, but it could have something to do with the fact that you have added a second extender and it is have hard time figuring what to use for apertures. It should still allow for manual exposure.
