Nice capture. You need to provide a bit of fill light to open shadow areas. I tweaked it a bit, but could only adjust exposure & contrast so much before highlights start to blow-out. Take a look at the Macro Equipment Set-Ups thread for inexpensive ideas for fill light.

Do you already own a speedlight? If so, an O-Flash 3/4-circle Fresnel prism attachment costs less than $30. Measure from center of speedlight lens to center of camera lens.

Last column: 6.1-inches = 155-mm, etc.

This is probably a dumb question, but - in the third column of the chart - how can the vertical distance between the center of the speedlite and the center of the lens be 61 inches, 63 inches etc? Should this be 6.1 inches, 6.3 inches etc. instead? Or are my old eyes as untrustworthy as my old brain?

You are absolutely correct. I can only copy the chart, not edit it, but I now labeled chart with correction example.

Excellent! I will use this corrected chart from now on. Did you add corrections yourself?

Yes I did. I downloaded the chart and carefully erased the old numbers and replaced them with the extra periods using CS4.

Some of them are not exactly lined up but it was the best I could do in a jiffy !
